HLSC 3473U – Prevention and Rehabilitation of Complex Chronic Conditions This course takes an integrated approach to the identification, determinants, prevention and management of complex chronic conditions. Societal, health care, workplace, physical and psychosocial barriers to rehabilitation process are identified and evidence informed approaches to enhance return to activities of daily living, increase quality of life, and facilitate return-to-work, where appropriate, are discussed. The course addresses the role of psychosocial risk factors in the development of disability. In particular, the role of self-efficacy, graded-activities, goal-setting, problem-solving, and motivation is explored.
